Getting Started

Set up CertifyForms in just a few minutes

Before you begin: Make sure you have a Google Forms quiz with scoring enabled and a Google account.

Step 1: Install CertifyForms

  1. 1 Open your Google Form in edit mode
  2. 2 Click the three dots (⋮) in the top-right corner
  3. 3 Select "Add-ons" → "Get add-ons"
  4. 4 Search for "CertifyForms" and click "Install"

Step 2: Configure Your Form

Enable Quiz Mode

Your form must be configured as a quiz:

  1. Click the "Settings" gear icon
  2. Go to the "Quizzes" tab
  3. Toggle "Make this a quiz" to ON
  4. Set point values for your questions

Collect Email Addresses

Enable email collection so certificates can be sent:

  1. Click the "Settings" gear icon
  2. Go to the "General" tab
  3. Toggle "Collect email addresses" to ON

Step 3: Choose a Certificate Template

After installing CertifyForms, open it from the Add-ons menu:

  1. 1 Click "Add-ons" → "CertifyForms" → "Open CertifyForms"
  2. 2 Click "Choose from Gallery" to browse templates
  3. 3 Select a template or create your own in Google Slides

Step 4: Add Merge Tags

Personalize your certificate with merge tags:

This certifies that {{Name}} successfully completed
{{Course}} with a score of {{Score}}.
View all available merge tags

Step 5: Set Passing Score

Configure when certificates should be generated:

  1. In the CertifyForms sidebar, find the "Passing Score" setting
  2. Enter the minimum percentage required (e.g., 80%)
  3. Only participants who score at or above this threshold will receive certificates

Step 6: Test Your Setup

Important: Always test first!

Before sharing your form, submit a test response to ensure certificates generate correctly:

  1. Preview your form
  2. Submit a test response with a passing score
  3. Check your email for the certificate
  4. Verify all merge tags were replaced correctly

Your automated certificate system is now ready. Participants who pass your quiz will automatically receive their certificates via email.
